The Introductction of Topiramate
Topiramate (abbreviated as TPM) is a natural monosaccharide allantoic dextrose sulfide, which is a broad-spectrum antiepileptic drug with felbamate, lamotrigine and aminoglutethimide with more clinical applications in China, and can be used to control different types of epilepsy with good efficacy and pharmacokinetics, but it can cause cognitive impairment and neurotoxicity in children and rapid dosing, and is prone to promote kidney stones. Its basic structure is pyranofructose sulfamate, which is different from other antiepileptic drugs.
Topiramate(TPM) has multiple antiepileptic mechanisms, blocking voltage-dependent sodium channels, enhancing GABA activity at γ-aminobutyric acid A (GABAA) receptors, blocking glutamate AMPA receptor activity, in addition to mild carbonic anhydrase inhibitor effects.
The results of double-blind, placebo-controlled studies in Europe and the United States have confirmed that topiramate add-on therapy is efficacious and safe in many types of refractory epilepsy and is well tolerated. Topiramate monotherapy has now been initiated and has also achieved good efficacy, with 62% of patients experiencing complete disappearance of seizures.
Topiramate is a white crystalline powder with a bitter taste. It is very soluble in alkaline solutions with a pH of 9-10 such as sodium hydroxide or sodium phosphate, and is readily soluble in aceton*, chloroform, dimethyl sulfoxide and ethanol. The solubility in water is 9.8 mg/mL, and the pH of its saturated solution is 6.3.
The Application & Function of Topiramate
Topiramate is generally used as an adjunct to antiepileptic drugs and is effective for simple and complex seizures with generalized tonic clonic seizures, as well as for the treatment of infantile spasms. Topiramate is characterized by good long-term efficacy, no significant drug resistance, and antiepileptic at high doses when used alone.
The primary mechanism of action of Topiramate is to block the spread of epileptic seizures rather than to prevent them, and TPM has been found to exert its antiepileptic effects through a variety of mechanisms, including:
1, blocking voltage-dependent sodium channels thereby reducing the duration of epileptic discharges and the number of action potentials generated per discharge.

Recent studies suggest that blockade of L-type high-voltage dependent calcium channels may be one of the most important mechanisms of action of topiramate against epilepsy.
