The Introduction of Artesunate

Artesunate is made from the active ingredient artemisinin, which is extracted from the unique Chinese Artemisia herb, and then chemically synthesized to act mainly on the asexual body of Plasmodium vivax red end stage.


Artesunate is a commonly used antiparasitic drug, a derivative of reduced artemisinin succinate monoester with sesquiterpene lactone structure, which is more potent than artemisinin against Plasmodium and also effective against antichloroquine strains. Artesunate acts on Plasmodium falciparum erythrointernal stage lysosomes, inhibiting cytochrome oxidase by reducing artemisinin, and achieving the function of inhibiting cytochrome oxidase of Plasmodium superficial membrane a food vesicle membrane and mitochondrial membrane system, thus blocking the supply of host cell plasma as nutrition. It has a strong killing effect on the asexual body of Plasmodium, and is effective in controlling malaria attacks rapidly, and has good effect on cerebral malaria, falciparum malaria and intermittent malaria, and is also effective on chloroquine-resistant strains of falciparum malaria. It is particularly effective against cerebral malaria, which can rapidly control malaria attacks, but is ineffective against gametes of P. falciparum.

 

The mechanism of action of artesunate is that artemisinin acts first on the food vesicle membrane, epithelium, and mitochondria of Plasmodium. This is followed by the nuclear membrane, endoplasmic reticulum, and, in addition, some effect on the chromatin in the nucleus. Since artesunate acts on the food vesicle membrane first, it blocks the nutrient uptake. It makes Plasmodium starve of amino acids sooner. Rapid formation of autophagic vesicles. And continuously excreted out of the body. A large amount of cytoplasm is lost and death occurs.

 

The Application & function of Artesunate

Artesunate can kill Plasmodium in the intraerythrocytic stage and achieve antimalarial effect, thus it can be used for all kinds of malaria patients, especially for the rescue treatment of patients with chloroquine- and piperaquine resistant falciparum malaria and aggressive cerebral malaria, with high antimalarial activity, fast fever reduction, short protozoal transfer time and low toxic side effects. It is the drug of choice for the treatment of severe malaria.


Artesunate is effective against parasites, such as liver flukes, and has also shown cytotoxic effects against different types of tumor cell lines.


Artesunate acts on the electron transport chain, generating local reactive oxygen species and causing depolarization of mitochondrial membranes. It inhibits TNF-induced production of pro-inflammatory cytokines by inhibiting NF-κB and PI3 kinase/Akt signaling pathways in human rheumatoid arthritis fibroblast-like synoviocytes.
