The Introductction of Ursodeoxycholic Acid (UDCA)
Ursodeoxycholic acid is a white crystalline powder, odorless and bitter in taste. It is soluble in ethanol, insoluble in chloroform; soluble in glacial acetic acid and dissolved in sodium hydroxide test solution.
Ursodeoxycholic acid is a chemical preparation of natural bile acid isolated from bear bile and is a differential stereoisomer of goose deoxycholic acid, with litholytic action and efficacy similar to that of goose deoxycholic acid, but with a shorter course and smaller dose. It exists in the bile in combination with taurine in the body, is a hydrophilic bile acid, and is a cholesterol stone dissolving agent.
Ursodeoxycholic acid can reduce the secretion of cholesterol by the liver, lower the saturation of cholesterol in bile, promote the secretion of bile acids, increase the solubility of cholesterol in bile, and dissolve cholesterol stones or prevent the formation of stones.
Ursodeoxycholic acid can increase bile secretion and relax the sphincter of the bile ducts to produce a cholagogic effect, which is conducive to the discharge of stones.
The Application & Function of Ursodeoxycholic Acid (UDCA)
Ursodeoxycholic acid reduces the activity of β-hydroxy-β-methylglutaryl coenzyme A (HMG-CoA) reductase, the rate-limiting enzyme of cholesterol synthesis in the liver, resulting in inhibition of cholesterol synthesis.
Ursodeoxycholic acid forms a stable liquid crystalline suspension with cholesterol, which unsaturates cholesterol in bile, thereby promoting the dissolution of cholesterol in stones by precipitation.
Ursodeoxycholic acid also inhibits the absorption of cholesterol from the intestine.
Ursodeoxycholic acid also antagonizes the cytotoxic effect of endogenous hydrophobic bile acids and protects hepatocyte membranes.
Ursodeoxycholic acid also inhibits apoptosis, inhibits inflammation, scavenges free radicals and is an antioxidant.
After oral administration, ursodeoxycholic acid is mainly absorbed in the upper jejunum by passive diffusion and in the ileum by active transport, with a large first-pass effect. 50%-75% of the oral dose is taken up by the liver; it is mainly distributed in the liver, intestine and plasma, and the plasma protein binding rate is 96%-99%. The concentration of ursodeoxycholic acid in bile shows a dose-dependent increase; at a dose of 20-30 mg/(kg-d), the concentration in bile exceeds 60%, and the best therapeutic effect can be achieved. It is combined with glycine and taurocholic acid in the liver and metabolized by enteric bacteria in the colon, with a small proportion of metabolites excreted in the urine and most excreted in the feces. The biological half-life is 3.5 to 5.8 days when taken orally.
