The Introductction of Furosemide

 

Furosemide, also known as furanilic acid, tachyphylaxis, tachyphylaxis, diuretics, diuretic sulfonamide, abdominal anhydrous acid, is a high efficiency diuretic of the aminosulfonyl group acting on the ascending medullary branch of medullary collaterals, which has a strong and transient diuretic effect and can increase the excretion of water, sodium, chloride, potassium, calcium, magnesium and phosphate. It is used clinically to treat cardiac edema, renal edema, cirrhotic ascites, peripheral edema caused by dysfunction or vascular disorders, and to promote the excretion of upper urethral stones. Intravenous administration can treat cerebral edema, and also accelerate the excretion of toxic substances in the case of cerebral edema drug poisoning.

 

Furosemide is white or off-white crystalline powder. Melting point 206℃. Soluble in aceton*, methanol, dimethylformamide, slightly soluble in ethanol, insoluble in water. Odorless, almost tasteless.

 

Furosemide mainly inhibits the reabsorption of Na+ and Cl- in the medullary and cortical parts of the ascending thick segment of medullary collaterals, promotes the excretion of sodium, chloride and potassium and affects the formation of high osmotic pressure in the renal medulla, interferes with the concentration and dilution process of urine, and increases urine volume. It can inhibit the activity of prostaglandinolytic enzyme and increase the content of prostaglandin E2, which has the effect of vasodilatation, and also has the effect on proximal tubule and glomerular filtration, which can increase renal blood flow and adjust the distribution of blood flow in the kidney, so that the medullary blood flow is reduced and the blood flow in the superficial cortex is increased, which promotes diuresis with rapid and powerful effect, and is mostly used in cases where other diuretics are ineffective.

 

It should be noted that when furosemide diuresis, the excretion of HCO3- does not increase due to the increase of Cl-, Na+, K+ and H+ excretion in the urine, and long-term repeated dosing or large amount of dosing can trigger hyposaline syndrome, hypochlorhydria and hypokalemic alkalosis.

Indication symptoms

  • Edematous diseases including congestive heart failure, liver cirrhosis, renal diseases (nephritis, nephropathy and acute and chronic renal failure due to various causes), especially when the application of other diuretics is not effective, the application of this class of drugs may still be effective. Combined with other drugs to treat acute pulmonary edema and acute cerebral edema, etc.
  • Hypertension is generally not used as the first choice for the treatment of primary hypertension, but this class of drugs is particularly suitable when thiazides are not effective, especially when accompanied by renal insufficiency or the development of hypertensive crisis.
  • Prevention of acute renal failure is used for various causes of inadequate renal perfusion, such as water loss, shock, poisoning, anesthesia accident and circulatory insufficiency, etc. Timely application while correcting blood volume deficiency can reduce the chance of acute tubular necrosis.
  • Hyperkalemia and hypercalcemia.
