The Introduction of Loperamide Hydrochloride

Loperamide hydrochloride is a common drug used to treat diarrhea, either as an antidiarrheal, or as a long-acting antidiarrheal drug, mostly for the treatment of acute and chronic dysentery. It is used on opioid receptors in the intestinal wall to block the release of acetylcholine and prostaglandins, thus inhibiting intestinal peristalsis and prolonging the retention of intestinal contents; it increases the tone of the anal sphincter, thus inhibiting fecal incontinence and urgency, and can also be used in patients undergoing anorectal surgery. At the same time, its high affinity with the intestinal wall and obvious "first-pass metabolism" make it hardly enter the systemic blood circulation.

Loperamide hydrochloride is a white or off-white crystalline powder with a chemical structure similar to that of dinoprostone and has no effect on the central nervous system at therapeutic doses. Its effect on intestinal smooth muscle is similar to that of opioids and defenofibrate, which can inhibit the contraction of intestinal smooth muscle and suppress intestinal peristalsis. By prolonging the residence time of food in the small intestine, it promotes the absorption of water, electrolytes and glucose, and has a significant inhibitory effect on intestinal hypersecretion caused by intestinal toxins, but does not affect gastric acid secretion at therapeutic doses.

The Application & function of Loperamide Hydrochloride

Loperamide hydrochloride is used for acute diarrhea as well as chronic diarrhea caused by various etiologies, such as ulcerative colitis, cologne, non-specific colitis, irritable bowel syndrome, short bowel syndrome, etc.

Loperamide hydrochloride is especially suitable for chronic functional diarrhea where the clinical application of other antidiarrheal drugs is not effective. It is also effective for diarrhea caused by partial gastrectomy and hyperthyroidism.

When loperamide hydrochloride is taken in overdose, symptoms of toxicity such as drowsiness, constipation, muscle tension, pupil narrowing and brady respiration may occur. Naloxone may be used to detoxify the patient and the patient should be observed for at least 48 hours in order to detect the central nervous system depression in time.
