The Introductction of Norepinephrine
Norepinephrine, a catecholamine, is a strong alpha agonist and also agonizes beta receptors. The agonism of α receptors can cause extreme vasoconstriction, increase blood pressure and coronary blood flow; the agonism of β receptors can strengthen myocardial contraction and increase cardiac output. When the dose is 0.4μg/kg per minute by body weight, β-receptor agonism is the main cause; when a larger dose is used, α-receptor agonism is the main cause. α-receptor agonism causes a wide range of vasoconstriction, with the skin, dipstick vessels, glomerulus being the most obvious, followed by brain, liver, mesentery, skeletal muscle, etc. Following cardiac excitation, the myocardial metabolite adenosine increases, and adenosine can promote coronary artery dilation. The cardiac manifestations of α-receptor excitation are mainly increased myocardial contractility, accelerated heart rate, and increased cardiac output; excessive elevation can cause reflex slowing of the heart rate, while the total peripheral resistance increases, and thus the cardiac output can be reduced. Excessive or prolonged use can cause capillary constriction and leakage of body fluid, resulting in a decrease in hemolysis.
Compared with norepinephrine and epinephrine, isoprenaline is a synthetic product whose chemical structure is the replacement of the hydrogen atom on the amino group of norepinephrine by isopropyl, and is a classical β1 and β2 receptor agonist.
The Application & function of Norepinephrine
Norepinephrine is used for the treatment of hypotension caused by acute myocardial infarction, extracorporeal circulation, pheochromocytoma resection, etc.
In shock or hypotension due to hypovolemia, norepinephrine is used as an adjunctive therapy to supplement hemolysis volume in emergency to bring back blood pressure to temporarily maintain cerebral and coronary perfusion; until the hemolysis volume supplementation therapy works.
Norepinephrine can also be used to treat hypotension during intravertebral block and to maintain blood pressure after resuscitation from cardiac arrest.
