The Introductction of Erythromycin

 

Erythromycin (EM, Er) is produced by streptomycin and is an alkaline antibiotic. It includes Erythromycin A (ErA), Erythromycin B (ErB), Erythromycin C (ErC), Erythromycin D (ErD), Erythromycin E (ErE) and Erythromycin F (ErF). Its free base for oral use, lactobionate can be used for injection, belongs to the macrolide narrow-spectrum antimicrobial agents, commonly used in penicillin-resistant Gram-positive bacterial infections and alternatives to penicillin allergy, the antibacterial spectrum is similar to that of penicillin.

 

Erythromycin is white or off-white crystalline powder; odorless, bitter taste; slightly moisture-attracting. Weakly alkaline, slightly soluble in water (about 0.2%), easily soluble in organic solvents. Unstable under acidic conditions.

 

Erythromycin is a gastric actin receptor agonist, the dimethylamine group and 14-membered lactone ring on the glycosidic chain of erythromycin molecule have a similar spatial structure to gastric actin, which can be used as a gastric actin receptor agonist to produce a mimic gastric actin effect and induce the early onset of MMC phase III contraction in the fasting state, causing gastric and duodenal smooth muscle contraction. The gastrokinetic effect of erythromycin is dose dependent. Low doses of erythromycin induce gastric contractions similar to spontaneous MMC, while high doses of erythromycin induce strong irregular gastroduodenal contractions inducing vomiting, which is the reason for the gastrointestinal reactions caused by erythromycin. Therefore, erythromycin should not be used in excessive doses for pro-gastric motility.

Erythromycin has the following effects:

3. Inducing gastrointestinal tract migrating complex movement; promoting colonic movement and gallbladder contraction, etc.
Therefore, it can effectively treat gastrointestinal diseases.

 

Clinically, erythromycin is often used for penicillin-resistant Gramnegative bacterial infections and patients allergic to penicillin, for infantile pneumonia and colitis caused by Legionella pneumonia, Mycoplasma pneumonia, Chlamydia trachomatis, skin soft tissue infections, and for respiratory tract infections caused by Streptococcus hemolyticus and Streptococcus pneumoniae, and for diphtheria carriers, the efficacy of this product is enhanced when combined with diphtheria antitoxin.
