The Introductction of Lidocaine Hcl:

 

Lidocaine Hcl salt, an amide local anesthetic, has anti-inflammatory properties in vitro and in vivo, possibly due to an attenuation of pro-inflammatory cytokines, intracellular adhesion molecule-1 (ICAM-1), and reduction of neutrophils influx.Target: Lidocaine is a common local anesthetic and antiarrhythmic drug. Lidocaine is used topically to relieve itching, burning and pain from skin inflammations, injected as a dental anesthetic or as a local anesthetic for minor surgery.

 

The Application & Function of Lidocaine Hcl:

 

The product is a local anesthetic and anti-arrhythmic drug. It is mainly used for infiltration anesthesia, epidural anesthesia, surface anesthesia (including mucosal anesthesia during thoracoscopy or abdominal surgery) and nerve conduction block. The product can be used for acute myocardial infarction ventricular premature beat and ventricular tachycardia, can also be used for digitalis poisoning, cardiac surgery and cardiac catheterization caused by ventricular arrhythmias. This product is usually ineffective for supraventricular arrhythmias.

 

Application:

 

Anesthesia

(1) Adult dosage:
① Surface anesthesia: 2%-4% solution should not exceed 100mg at a time. Do not exceed 4.5mg/kg at one dose (without epinephrine) or every 7mg/kg (with 1:200,000 concentration of epinephrine).
② Sacral canal block for labor analgesia: 1.0% solution, limited to 200mg.
③ Epidural block: 1.5-2.0% solution for thoracolumbar, 250-300mg.
④ Infiltration anesthesia or intravenous regional block: 0.25%-0.5% solution, 50-300mg.
⑤ Peripheral nerve block: brachial plexus (unilateral) 1.5% solution, 250-300mg; Dental 2% solution, 20-100mg; Intercostal nerve (each) with 1% solution, 30mg, 300mg limit; For paracervical infiltration, 0.5%-1.0% solution, 100mg on the left and right sides; Paravertebral spinal nerve block (each) with 1.0% solution, 30-50mg, up to 300mg; Pudendal nerve should be treated with 0.5%-1.0% solution, 100mg on the left and right sides.
Sympathetic ganglion block: cervical stellate nerve with 1.0% solution, 50mg; 1.0% solution for waist hemp, 50-100mg.
⑦ One limit, no adrenal 200mg (4mg/kg), adrenal 300-350mg (6mg/kg); Iv regional block, maximum 4mg/kg; For therapeutic intravenous infusion, the first initial dose is 1-2mg/kg, the maximum dose is 4mg/kg, the adult intravenous infusion is limited to 1mg per minute; Give the drug repeatedly with an interval of no less than 45-60 minutes.
(2) The usual dosage of children: varies with the individual, the total dose of one dose shall not exceed 4.0-4.5mg/kg, 0.25%-0.5% solution is commonly used, and 1.0% solution is used in special cases.


Antiarrhythmic


(1) often use
① intravenous injection: 1-1.5mg/kg body weight (generally 50-100mg) for the first load for 2-3 minutes, if necessary, repeat intravenous injection once or twice every 5 minutes, but the total amount within 1 hour shall not exceed 300mg.
② Intravenous infusion: Generally, 5% glucose injection is prepared into 1-4mg/mL drug infusion or infusion pump. After using the load, it can continue to be maintained at the rate of 1-4mg per minute, or at the rate of 0.015-0.03mg/kg body weight per minute by intravenous infusion. In the elderly, heart failure, cardiogenic shock, reduced hepatic blood flow, and hepatic or renal dysfunction, the dosage should be reduced to 0.5 to 1mg per minute. This product can be administered intravenously with 0.1% solution, not exceeding 100mg per hour.
(2) the Polaris
Maximum iv loading within 1 hour is 4.5mg/kg body weight (or 300mg). Maximum maintenance is 4mg per minute.
